Retrospectives

This is where incidents, mistakes, and operational failures are collected and recorded after the urgent response is over.

Purpose

The goal is to make it visible what happened, what was learned, and what changed afterward. The emphasis is not on self-criticism, but on keeping failures in a form that can improve the next round.

What gets recorded

  • A timeline from the start of the issue to its resolution

  • Root causes and contributing factors

  • Impact on users and how long it lasted

  • Changes made to prevent the problem from happening again

Current state

There may be long periods with no public post-mortems. That does not mean operations are perfect; it only means nothing has been published in this format yet.
